take financial responsibility for the required sport, uniform, and fundraising fees

assume all liability for any injuries that may incur during participation

submit payment using the online payment option (add 2.75% processing fee)

read and agree to the contents of CYO Athletics Parent Code of Conduct

read and agree to the contents of CYO Athletics - Concussion Information Sheet

read and agree to the contents of Athletic Waiver of Liability Form
